Change In Motion specializes in evidence-driven, person-centered evaluations and interventions to support aging in place, fall prevention, home accessibility, and improving fatigue and arthritis pain by modifying daily activities and home environments.

All evaluations utilize standard objective assessments and a checklist of over 400 points.

Intervention includes education on local resources and organizations that support patient needs.

Change In Motion accepts Medicare Part B and uses a sliding scale for all patient fees.

Change In Motion has no financial affiliations with any additional services, products, or networks.

OT home evaluation referrals can be faxed to 484-705-2036 or emailed to OT@ChangeInMotionNJ.com. Please include patient contact information and reason for referral.
Qué esperar
Change In Motion will contact the patient within three business days of receiving the script, and the home evaluation will be scheduled promptly.
Home evaluations last 2-5 hours. A summary of the evaluation, findings, and recommendations will be faxed to the referring provider within five business days after the evaluation. Follow-up visits and phone calls will be scheduled with the patient as needed; summaries of follow-up visits will be faxed to the referring provider.
